Paperwork Checklists

Use these Checklists as your guide to be sure your application package is complete before you email to the paperwork review team.

Pages 3-5 (sign and date pages 4, notarization no longer required). Make sure all information is filled in. If any of the six legal questions are checked off “yes”, please fill out Convictions form CG719C.

Copy of valid TWIC (or receipt that you have applied)
Proof of Citizenship. Copy of birth certificate or current passport.
Certificates for license requested: (i.e. OUPV, Upgrade to Master etc)
CPR/First Aid certificate: (ensure course is CG approved and less than 12 month old)
Sea Service Forms (CG719S)

Fill out the Small Vessel Sea Service form CG719S. Forms need to be fully completed, including all math. If using your own vessel, provide proof of ownership showing the vessel is on your personal name. Company letters will work as long as they include full information as on sea service forms, are on letterhead, and signed by a company official.
Required Sea time for Original OUPV = 360 Days. Of that total, 90 days must be within the past 3 years.
Required Sea time for Master (25, 50, 100GT) = 720 Days. Of that total, 180 days must be within the past 3 years.

Federal DOT 5 Panel only. CG719P form signed by an approved MRO, or letter from approved maritime consortium program, or letter from maritime employer if part of an employer consortium.

$145 paid on pay.gov website (“officer original” $100, plus “pay issuance fee now” $45).

RENEWAL APPLICATIONS

Pages 3-5 (sign and date pages 4 and 5). Make sure all information is filled in.
Original & Renewal License Application Packet

Copy of valid TWIC (or check off box on top of page 4 of above application).
Copy of current Merchant Mariner Credential. Please send all copies of all pages with license information.
Sea Service or license renewal course certificate.
If using sea time, fill out the Small Vessel Sea Service form CG719S. You must show at least 360 days in the past 5 calendar years, with at least 90 days in the past 3 calendar years. If using your own vessel, provide proof of ownership showing the vessel is on your personal name. A signed letter on company letterhead from your employer can also be used.
Drug Test (CG719P)
- Form signed by an approved MRO, or letter from approved maritime consortium program, or letter from maritime employer if part of an employer consortium.
Application for Medical Certificate (CG719K) - or copy of a valid medical certificate.
Receipt for pre-paid Coast Guard fees
- $95 paid on pay.gov website (officer renewal $50 plus pay issuance fee now $45).
